No patient relationship

Using the Site, submitting a form, or emailing us does not create a therapist-patient relationship. That relationship begins only after you complete our intake process and we agree to provide care.
 

Services and service area

Driven Physical Therapy and Wellness, LLC provides in-home physical therapy in Wake, Harnett, and Johnston Counties, North Carolina. We are licensed in North Carolina and provide care only where we are licensed. Availability depends on location and scheduling.
 

Payment

Driven Physical Therapy and Wellness, LLC is a cash-based (self-pay) practice. We do not bill Medicare, Medicaid, or private insurance. Payment is due at the time of service. On request, we can provide a receipt or superbill you may submit to your insurer or use with an HSA or FSA. We cannot guarantee reimbursement. If you are not using insurance, you have the right to a Good Faith Estimate of expected charges.
 

Cancellations and missed visits

Because we travel to you, please give at least 24 hours notice to cancel or reschedule. 
 

Use of the Site

You agree not to misuse the Site, including by attempting to gain unauthorized access, interfering with its operation, or submitting false or harmful information.
 

Intellectual property

All content on the Site, including text, graphics, logos, photos, and videos, belongs to Driven Physical Therapy and Wellness, LLC or its licensors. You may not copy or reuse it without written permission.
